Roads to Community Living

Roads to Community Living provides support and services to people interested in moving from a Residential Habilitation Center, nursing facility or hospital back to the community. This can include helping people and their family or guardian make a well-informed decision regarding where to live and what support would be needed to live successfully in the community. Enhanced funding for additional help is available for one year after a person moves into the community. These extra services and flexible funding are tailored to each person’s needs and desires. I live in a nursing facility. Can I get additional services? What if I want to move to the community?

Pre-Admission Screening and Resident Review

PASRR (pronounced Pass R) is a screening and evaluation process for people with IDD or related disability who have been referred for nursing facility care. Part of the process includes a PASRR Assessor who helps individuals connect with services beyond the standard nursing facility care while they are residing in the nursing facility. This program can help people find needed supports if they are interested in returning to their communities such as Community Engagement services, the Family Mentor Program and Roads to Community Living.
