Discharge planning begins at admission. The interdisciplinary team will develop targets based on your needs to meet your discharge goals.
Washington State operates the following intermediate care and skilled nursing facilities for individuals with intellectual disabilities:

Lakeland Village in Medical Lake (ICF and Nursing Facility)

Fircrest School in Shoreline (ICF and Nursing Facility)

Yakima Valley in Selah (Nursing Facility)
Each facility is federally certified to provide 24/7 support to adults age 21 years or older. Clients age 16-20 may be eligible if services are not available in the community to meet their needs.
