Keeping COPD Patients Healthy at Home: A Learning Series for Caregivers (2 Sessions)

Description: The Center for Medicare and Medicaid Services is working to decrease hospital readmission for Medicare beneficiaries. The Respiratory Health department at Legacy Health Salmon Creek Hospital has found that patients living with COPD experience disproportionate rates of hospital readmission. Lack of knowledge and skills around caring for COPD patients, and lack of training for caregivers on this important topic, have been identified as key factors contributing to high readmission rates in this population.
The Care Coordination team at Comagine Health has partnered with the Respiratory Therapy team at Legacy Salmon Creek to create a 2-part training for all direct care workers in state-licensed assisted living facilities, adult family homes, respite care providers, community residential service providers, and any other direct care staff who provide home or community-based services to the elderly or persons with disabilities. This training will increase caregivers' capacity to care for COPD patients and will focus on key strategies for keeping COPD patients healthy, including recognizing symptoms of COPD onset and exacerbation, proper nutrition for COPD patients, and proper respiratory equipment use and maintenance. The first session of the training will focus on the COPD disease course and patient needs. The second session of the training will focus on tactical skills around equipment.
